Protection of Personal Information Act, 2013 (POPIA)
CLIENT / DONOR / VOLUNTEER DECLARATION
I, as a donor, volunteer, beneficiary, programme participant, service provider, employee, partner or stakeholder of Atlantic Fire and Rescue NPC trading as Atlantic Fire Services (“Atlantic Fire Services”), hereby acknowledge that my personal information, and in certain circumstances special personal information, may be required by Atlantic Fire Services in order to fulfil its mandate and deliver its services.
I agree to provide such information requested by Atlantic Fire Services on the express understanding that:

This constitutes my consent, as required under Section 11(1)(a) of the Protection of Personal Information Act 4 of 2013 (“POPIA”), for Atlantic Fire Services to collect, process, store and use my personal information for legitimate operational purposes.
2.
Employees, authorised volunteers, contractors and representatives of Atlantic Fire Services may access my information solely for purposes directly related to the activities, programmes, emergency services, fundraising initiatives, training programmes or other matters in which I participate or am involved.
3.
Atlantic Fire Services is authorised, where legally required or operationally necessary, to disclose my personal information to:
Government departments and regulatory authorities;
The South African Revenue Service (SARS);
Payment gateway providers and financial institutions;
Auditors, accountants and legal advisors;
Emergency service partners and disaster management authorities;
Service providers assisting Atlantic Fire Services in fulfilling its mandate;
Any other authorised third parties directly involved in services or activities relating to me.
4.
I acknowledge that Atlantic Fire Services may communicate with me regarding:
Community programmes;
Emergency response initiatives;
Volunteer opportunities;
Fundraising campaigns;
Organisational updates;
Training opportunities;
Newsletters and impact reports.
I hereby consent to receiving such communications. I understand that I may opt out of non-essential communications at any time.
5.
Atlantic Fire Services will retain and store personal information in accordance with POPIA, applicable legislation and any regulatory requirements relevant to the organisation’s operations and nonprofit status.

Atlantic Fire Services has implemented appropriate information security measures, data privacy policies, cyber security controls and internal procedures to safeguard personal information against unauthorised access, loss, misuse, alteration or disclosure.
While Atlantic Fire Services takes reasonable precautions to secure information, I acknowledge that it remains my responsibility to protect my own devices, internet connections and email systems against cyber risks.
7.
Atlantic Fire Services respects the confidentiality and privacy rights of all individuals and undertakes to process personal information lawfully, reasonably and transparently.
8.
I confirm that:
a) I have had an opportunity to review the Atlantic Fire Services Privacy Policy, PAIA Manual and POPIA-related policies.
b) I have had an opportunity to ask questions regarding the collection, processing, storage and sharing of my personal information.
c) I understand why my information is being collected and how it will be used.
d) I voluntarily consent to the collection, processing, storage and lawful sharing of my personal information by Atlantic Fire Services in fulfilment of its mandate to provide fire prevention, emergency response, community resilience, volunteer development and related nonprofit services.
